Institutional Effectiveness Request Form

The Office of Institutional Effectiveness (OIE) will fulfill requests for institutional data and surveys, such as a list of UB students or graduates of specific programs along with other student information that is not prohibited by FERPA guidelines for all programs, schools, and departments.

To facilitate a prompt and complete a request, provide and include as many of the specific data points listed in the form below.

Please keep in mind that most requests take a minimum of two weeks, with priority given to those related to external agency requirements (e.g., State of CT, NECHE). It is imperative that you submit your request as soon as possible. Thank you for understanding as we prioritize the queue.
